Overview

Our health (medical, dental and vision) insurance provider is Blue Cross Blue Shield.

Blue Cross Blue Shield is widely recognized as the leading national health insurance carrier in the United States. They are known for consistently having high-quality healthcare services, and an expansive network of providers, hospitals, and medical professionals. 

We offer three leading plans with Blue Cross Blue Shield: 

1. an HSA option; 

2. a low deductible PPO 500; and 

3. a mid-deductible PPO 1500 

All plans include the following: 

• An overall excellent healthcare offering 

• A large, national network that can serve staff in all states

• In-network and out-of-network coverage

• Mental health coverage

• Highly or fully subsidized birth control

• Gender affirmative care

• Fertility coverage (linked to details) 

• Wellness subsidy

• Travel assistance for abortion care (in states where it is illegal)

Register on the Anthem BCBS web portal (https://www.anthem.com/login/) to create an account. Within your account on the portal, you can: keep your records up to date; print a temporary ID card; request a new card if your card gets lost or misplaced; find a physician; and access other general information about your insurance. 

To log into the BCBS portal, go to https://www.anthem.com/login/ and register using your Member ID and Group ID information. If you do not have this information, please contact HR@orchestraco.com.

To download a temporary ID card:

1. Login to https://www.anthem.com/login/ 

2. Once you’re signed in, click on “Request ID Card” and “Print” to get a copy of your card now and/or “Submit” to receive a new card in the mail.

3. If you choose just medical insurance, you will receive a medical card from Anthem. If you also have dental and vision, this will be included on one card. You can print an ID card through the Anthem online portal.

To find an in-network doctor, specialist, or facility, visit: Anthem Find Care

If you are not logged in (Search as a Guest):

1. Select “Basic Search as a Guest.”

2. Choose “Medical.”

3. Enter your state.

4. Under “What type of plan do you want to search with?” select “Medical – Employer-Sponsored.”

5. Choose your plan/network:

   • New York residents: select Blue Access (PPO) Network

   • All other locations: select National PPO (BlueCard PPO)

6. Click Continue.

7. Enter your ZIP code and select “Types of Providers.”

8. Review your search results to find in-network care.
   
   

If you are a registered member:

Log in directly at Anthem.com or use the Sydney Health app.
You’ll automatically be directed to your provider network and pharmacy formulary based on your plan.

You may find the 2025/2026 Annual Medicare Part D notice here. This notice contains important information about your prescription drug coverage and how it relates to Medicare Part D.

• Who is eligible for Medicare Part D?

  • Anyone eligible for Medicare Part A or enrolled in Medicare Part B (typically individuals 65 or older, and certain younger individuals with disabilities).

  • Individuals with End-Stage Renal Disease (ESRD).

  • Medicare beneficiaries under 65 who are eligible due to a disability.

• Dual-eligible individuals enrolled in both Medicare and Medicaid.
